Fine Tuning Your PDPM Metrics: Nursing Documentation

Skilled nursing documentation is extremely important to ensure accurate and complete portrayal of the resident’s condition and interventions provided to care for the resident. In this session, we will explore why skilled nursing documentation is important, discuss the differences between skilled documentation expectations under PPS versus under PDPM, review the four main required principles to utilize when completing Medicare charting, and explore what each Registered Nurse Manager needs to know when skilling a resident. Skilled nursing documentation examples will be reviewed, and our client partners will receive a documentation checklist tool.

Objectives:

1.       The participant will have a better understanding of the importance of skilled nursing documentation;

2.       The participant will improve their knowledge on the differences between skilled nursing documentation expectations under PPS versus under PDPM;

3.       The participant will learn the four main required principles to utilize when completing Medicare charting;

4.       The nurse participant will improve their ability to portray skilled nursing interventions in the nursing documentation to better support PDPM nursing clinical categories.

 

Target Audience: MDS Nurses, Director of Nursing, Assistant Director of Nursing, Nursing Home Administrators

Teaching/Learning Methods: Lecture, visual presentation materials, case examples/case studies, question and answer

Speaker Information:
Verona Bair, RN, RAC-CT, Clinical Reimbursement Specialist, Functional Pathways


Verona is a Registered Nurse with over 45 years of experience working in multi-roles in the long-term care industry. She is currently The Clinical Reimbursement Specialist with Functional Pathways. She has spent the previous ten years as an independent consultant working as a DON/MDS Coordinator in buildings nationwide. As a DON she specialized in focus buildings; those struggling with compliance and helped obtain recertification for decertified facilities. As an MDS Coordinator, she has worked in large census buildings and in communities that had high Medicare utilization with rapid admission and discharge processes. In both roles, she focused on education related to adhering to regulation, policy and procedure, as well as reimbursement. Verona is passionate about developing the documentation and assessment skills of the front-line nurses. She wants to guide the MDS nurses by educating the detailed capturing of all suitable documentation to obtain the appropriate reimbursement for the care nursing is providing to the resident. As well as creating a bridge for nursing documentation to support therapy in a mutual goal to improve the residents’ quality of life and care.

 

Gina Tomcsik, LPTA, RAC-CT, Director of Compliance and Regulatory Strategy, Functional Pathways

Gina Tomcsik is a Licensed Physical Therapist Assistant with over 31 years of post-acute care experience. Gina has been with Functional Pathways for ten and a half years and currently serves as the Director of Compliance and Regulatory Strategy. In this role, Gina oversees the management of the denial and appeals process, internal auditing and monitoring, compliance with regulatory changes and reimbursement in both the inpatient and outpatient therapy settings, and adherence to rehabilitation standards, regulations, and delivery of therapy care. Gina has strong professional skills in documentation compliance and auditing, strategic planning, and team building. Gina passionately provides education in the areas of therapy documentation, billing, and coding at state health care association conferences and has lectured to Health Information Specialists at The American Health Information Management Association (AHIMA) conference as well as state and national health care conferences. Gina is active in The National Association for the Support of Long Term Care (NASL) and has met with Senators and Representatives on Capitol Hill in the repeal of the therapy caps among other Post-Acute Care interests. Gina is a member of the Health Care Compliance Association (HCCA), she currently holds membership with The American Association of Nurse Assessment Coordinators (AANAC) and is certified in Resident Assessment Coordination (RAC-CT).
