Managing and Learning from Payment Reform - July 21, 2019

As the leading rehabilitation therapy provider in the post-acute market, RehabCare has worked with thousands of Skilled Nursing Facilities over the years to guide them through various payment reform, the most recent being the Patient-Driven Payment Model (PDPM). Implementing innovative payment reform projects that drive high-value health care in their communities requires oversight and participation by every key aspect of the provider’s business, from clinical all the way to facilities management. In this session, we will explore how, by providing clinical, technical and regulatory assistance and fostering a learning network, RehabCare helped to support the work of Lifespace Communities and inform strategic conversations among its facility and other similar healthcare providers working to implement payment reform.

This session focuses on the importance of diligently managing the many details that comprise transition to new payment reforms, the importance of rethinking the traditional relationship between healthcare providers and vendors to one of true, consultative relationships; and the top-down lessons learned in helping providers transition to big payment model changes like PDPM.

Sara Hamm, PhD, Vice President of Successful Aging and Health Services, Lifespace Communities
Mary Van de Kamp, Chief Clinical Officer, RehabCare
Glenda Mack, Chief Operations Officer, RehabCare
